Peak & Dales Medical Partnership covers Bakewell Medical Centre and Tideswell Surgery, if you have a preference of which site you wish to be booked in to, please make this clear to the member of staff you are speaking to.

Our clinics have now changed to allow better access for you. Each clinic is now a mix of prebookable*, on the day and urgent appointment slots. A GP will look at your request and advise the best action for you.  Please provide as much detail as possible to ensure you receive the correct appointment.

If you are unable to complete the form yourself, our reception team will do this for you.  Please note that calls will take longer to answer initially. 

Urgent requests will be prioritised, so if you feel you must be seen the same day, as an emergency, please make this clear.

Appointments with our clinicians are timed, if you wish to discuss more than one issue / problem during your appointment; please advise thewhen requesting so we can determine whether you will need a longer appointment.

If there are aspects of the new system which you find difficult or unhelpful please do let us have your suggestions as to how we can improve these; bearing in mind we clearly have to work within the resources we have. We do realise that any system cannot please all our patients all the time.

Making Access easier 

 

Many patients have told us they cannot get an appointment when they need one.  Taking this information on board, we are moving to a triage model.  Your request for an appointment will be viewed by a GP, who will then deal with your request accordingly.  This may mean a same day appointment, a pre-booked appointment for another day, or dealt with electronically/by telephone. 

This move is to direct people to the right appointment at the right time. 

Nursing appointments and HCA appointments will still be self bookable.

Extended Access Appointments

We open late nights on Tuesdays at our Bakewell site.  Usually there is a Nurse Prescriber, Practice Nurse and Healthcare Assistant.

We also open every 5th Saturday (appointment only) 9-5pm 

We are working in collaboration with other GP Surgeries in the Derbyshire Dales to provide this service, so this may be a GP from another Practice.

Our receptionists will be able to advise which Practice the GP will be from for these appointments but not the specific GP.

Nurse Appointments

We have Nurse and HCA Appointments available at both sites.

These appointments are not currently available online; please contact the surgery by telephone to book.

Review Appointments

We are now operating all core services. We will be contacting patients to invite for annual reviews. 

A lot of what we do at Bakewell and Tideswell is preventing illness. Therefore, patients with long-term conditions need checking at routine intervals to make sure they are well. Patients are often confused about this and the language used. In medicine, chronic means long-term, not severe.

Home Visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling 01629 816636 before 11:00am. A GP will telephone in the first instance and plan the best treatment for you.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a Community Nurse if you are referred by your GP. You could also be visited at home by a Health Visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

The NHS App

If you’re a patient at our practice you can now use the NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.

You can use the NHS App to get health advice, book appointments, order repeat prescriptions, view your GP health record and more.

AirMid by TPP (SystmOnline)

TPP (SystmOne) have developed a new app – ‘Airmid’ – which provides patients with existing SystmOnline functionality plus lots more! Try the new Airmid app now. You can log in with your SystmOnline username and password.

Otherwise, Airmid allows patients to login using NHS login. An NHS login can be created directly via Airmid.

Cancelling or changing an appointment

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance (excluding weekends and public holidays). You can also text to cancel your appointment.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

If you need help with your appointment

Please tell us:

  • If there’s a specific doctor, nurse or other health professional you would prefer to respond
  • If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • If you need an interpreter
  • If you have any other access or communication needs

Chaperones

All patients are entitled to have a chaperone present for any consultation, examination or procedure, where they feel one is required. This chaperone may be a family member or friend. On occasions you may prefer a formal chaperone to be present, i.e. a trained member of staff.

Wherever possible we would ask you to make this request at the time of booking your appointment so that arrangements can be made and your appointment is not delayed in any way. Where this is not possible we will endeavour to provide a formal chaperone at the time of request. However, occasionally it may be necessary to reschedule your appointment.

Your healthcare professional may also require a chaperone to be present for certain consultations in accordance with our Chaperone Policy.
