Welcome to Peak & Dales Medical Partnership
We are confident that our website will provide clear and concise help and give our patients the information they require in an easy and convenient format. It has been designed with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us.
Our dedicated team are here to treat you as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription and book or cancel an appointment. Our patient advisors may direct you to more appropriate places for care, such as a pharmacist, a dentist, a physio or A and E should your condition be serious.

Patient Participation Group
Would you like to have more input and help us to improve?
We are always looking for people who are keen to join our patient participation group. The aim is for the group to act as a link between the surgery and patients. We want any changes to benefit our patients and are particularly keen to welcome teenagers as well as all other ages see our patient population equally represented.
What is their role?
The patient participation group consists of patients who wish to be involved in the local practice whilst taking an active role in developing local health services. The purpose of the group is to facilitate patients and the practice staff working together to share ideas to help improve the services offered at the practice in addition to sourcing out any local community services that would help enhance aspects of the patient's lives. The group will also have the responsibility of helping patients to take more responsibility for their own health, the group also offers an avenue for patients to have a say in how services are planned, developed and evaluated to foster a good working relationship with the practice staff and GP's.
What does the patient participation group do?
· Help improve on the experience of attending the surgery.
· Helps bring the attention of practice staff, the practice perspective of the level and standard of the services provided.
· Acts as a channel - communicating to patients re 1) how changes in the NHS will affect service provision and 2) information on the help available, support groups and networks.
· Helps improve patients overall experience of the practice.
